To request an appointment at an Imaging Associates clinic, simply complete our online booking form and one of our team members will be in contact to find you a suitable date and time.

Please ensure you have your referral with you, as you will be required to upload it for our team to complete your booking. We accept all referrals.

Please note: If you have more than one referral from your referring doctor and need to schedule multiple scans or procedures, please call us directly.

• A referral from your practitioner, GP, specialist or other healthcare professional
• Medicare card (if applicable)
• Healthcare card (if applicable)
• Pension card (if applicable)
• Department of Veteran Affairs card (if applicable)
• Any relevant previous images
